Once approved, your account will qualify for your patient discount AND you will have access to more products!

 

1. Please fill out all required info in the Patient Account Form.
2. We will then review and verify your account (if you apply outside of regular office hours, please allow 1 business day for account verification.)
3. Once approved, we will send a Customer Account Confirmation email.  Click the link in the email and you will be prompted to create a password.
4. You're all set!
